Grove Park & Northside Preschool Registration Form
**Early Special Education Program for eligible children ages 2-4. Call Grove Park Preschool at 434.799.6437 or Northside Preschool at 434.773.8301 for information on determining eligibility and enrollment requirements.
To register your child:
Times: Call to make an appointment
Locations: Grove Park Preschool or Northside Preschool
The following information must be provided:
- Child's Official State Certified Birth Certificate (If you do not have this you need to order NOW www.vdh.virginia.gov) The certificate issued by the hospital does not meet the state requirement.
- Income Verification (2021 Income Tax Return)
- Most recent Immunization Record including 4 year old boosters, as appropriate
- Schedule your child's School Entrance Physical. (It must be dated after August 10, 2021.)
- One proof of current residency documents must be provided. The child must reside in the city of Danville with either parent or guardian. Parent needs to show a current utility bill, bank statement, rental agreement, etc. and a picture ID card.)
- Photo identification of Parent or Legal Guardian
- At the time of registration, you will receive an appointment for your child to be screened for eligibility if you have provided the necessary documentation listed above as items #1, #2 and #5. Other items are needed prior to enrollment if found eligible. Otherwise, screenings will be scheduled when documentation is complete. When all preschool positions are filled, a waiting list will be initiated.

KINDERGARTEN ROUNDUP!!!
Will your child be five years old on or before September 30, 2022? If so, It’s time for KINDERGARTEN!!!
Parents should register their child in the school to which their child is assigned based on the school division's attendance zones. If parents are unsure about which school their child is to attend, they may call the School Board Office (799-6400). Registration forms can be secured from the schools or by accessing the linked documents below.
At the time of the registration, parents are requested to have:
- the child's official birth certificate
- One proof of residency (If an affidavit is provided, it must be approved by the Superintendent.)
- an immunization record
- a physical examination record (must be provided before the beginning of the 2022-2023 school year)

Entrance Guidelines

Age of Kindergarten Entrance:
Students enrolling for kindergarten shall have reached their 5th
birthday on or before September 30 of any school year.Mandatory Entrance Requirements:
The Commonwealth of Virginia mandates the following requirements
prior to entrance in public schools:- Physical Examination(for Kindergarten and elementary school)
An examination is considered if it was administered within 12 months
of starting school.
- Immunizations Requirements (K-12) A document from a
physician's office or Health Department with the following
immunizations must be shown to a school official prior to admission.

- DTP (diphtheria, tetanus, pertussis) - four doses with one dose
given after the 4th Birthday, a booster dose of Tdap vaccine is
required for all children entering the 6th grade
- DTP (diphtheria, tetanus, pertussis) - four doses with one dose

- OPV or IPV (polio) - four doses with one dose given after the 4th
birthday
- OPV or IPV (polio) - four doses with one dose given after the 4th

- MMR (measles, mumps or rubella) - minimum of two measles, two
mumps and one rubella

- Birth certificates must be shown to the appropriate school official
prior to admission. This is the certificate received from Richmond, not
given out at the hospital. If you do not have a birth certificate you
will need to fill out a Birth Certificate Affidavit and have it notarized
by the School Board.

- One proof of residency must be shown to the appropriate school
official prior to admission. Acceptable examples are: signed current
lease agreement or current mortgage statement, a current
water/gas/electric bill, current voter registration card, state or federal
tax return filed within the past 12 months, check stubs from current
employer with address (most recent two months), or current Virginia
driver’s license or identification card.
